Create a Rubic Scoring Tool

Make a tool from this Rubic [login to view URL] on the top have fields for: Writer name: Article Title: Additional Comments: Then show the Rubric for scoring. Remove the category for "Multimedia" There would not be a check box by each point....but one check per box. (an editor would use this tool to score each section), then they would press enter and it would go to a page that showed their score in each category, their name, the article title, Additional comments, the grading scale, and then their the final Score on the bottom. Give 1 point for a 1 score, 2 points for a 2 score, etc. press a button after you've checked the box, and get a score. 7-10 is a fail $0 11-14 is C $10 15-18 is a B $15 19-22 is a A $20 23-24 is a A++ $25 When the grader presses enter, I want the results to produce a unique url that we copy to give to the writer so they can see their grade. first produce this on your own system, then we'll move to our systems. php please.
