I have a relatively easy project that needs done.

Currently, I have customers subscribing to my small hosting service. they submit via an online form at http://signup.100for100.com.
I will receive this form via email. I will then open a doc file, with generic fields like DOMAINNAME, USERNAME, Expiration date. Which I use WORDS to do a FIND/REPLACE and replace all DOMAINNAME field with their new domain name etc.
Then I will email them this Welcome email, with all their login details etc.
Next, I will generate an e-invoice via WORDS and output to pdf file, using their signup form details e.g. name, company, address, package signedup, and amount received.
I will then email this to them with the welcome email.
I want to streamline this process.
My requirements are:
Create an online form, where I can easily enter the fields
user email 1 and email 2, domain name, user name login, password, date of signup, date of expiry
I can then click generate and it will update a template form with a preview. When ok, I click SEND and it will send the generated welcome email to user email 1, 2 and my admin email for filing.
when send, it will show, email sent.
Next, I will have a generate invoice button, where I can enter in fields
name, company, address, date, invoice number, description of product, fee, total (i can do the calculation manually and enter in the numbers myself).
I click Generate and the invoice preview is shown. If ok, it'll either send out a html page invoice or if possible, a pdf invoice using invoice number as filename, to email 1 email 2 and my admin email.
I am using Linux servers, running php + mysql.
I can provide all template in doc file, and you can modify to suit a web format.
