I have an internal sales tool which is currently in use and we need to clean-up the usability with rewriting a number of the PHP based forms.
I will be able to provide access to the code via a Github repository.
The from (which is probably 80% complete at the moment) needs to capture various details including client details, client requirements, allow for notes to be made and list all the services that they could be potentially interested in. I will provide all the contents for the form and a basic layout, you're job will be to create this form to allow capturing of this data. Basic data validation (only a small number of the fields are required and only basic sanity checking (to match DB constraints)), and insertion into a MySQL back-end.
An adapted version of this form will be required so the database entries can be updated through the life cycle of the sale.
Finally, basic reporting will be required so each agent can trivially get a big picture overview of any tasks which are currently outstanding.
There will finally be a small element of UX. The forms must be intuative and clean to use; I am not looking for a work of art. Usability is king.
There is a total of 3 PHP pages and an amount of MySQL smarts which will be needed.
This is being done more as an experiment and there is the opportunity for much more on-going work for the right person.
