Create form for use in MS Word

This project should be pretty easy for someone who has created forms. We'd like to create a form for our associates to use in MS Word that resctricts content in some sections and allows flexibility in others. The goal is to create something more rigid than an MS Word template. How will the product be used? Our associates attend lots of meetings that require rigorous notetaking. We assemble these notes at the end of the meetings and distribute them. We want to make sure certain pieces of information are always captured the same way (meeting speakers, meeting topic, etc.). We also want to have "blank" sections for note taking. We want to make sure that formatting cannot be changed and is as consistent as possible across operating systems (Mac and PC). We need to ensure that reassembly of all the "notes" documents can be done easily at the end of the process. For that reason, we might be cautious about using certain formats (for example, tables). We have mockups of what the "form" should look like as well as the final output that we can share. Please contact me with any additional questions. ## Deliverables 1) A fully functional form that can be used in MS Word on both Macs and PCs interchangeably. 2) Deliverables must be in ready-to-run condition 3) Documentation for all work performed for the purpose of making further upgrades or extensions easier and more efficient to complete. 4) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform The project needs to run on both Mac and PC platforms.
