add smtp details to existing php script

I have a php mail script that works perfect except my host has advised I need to add smtp details ito it. If you can the remove any other invalid code that would be great. My form must still be able to handle the html email and also have the auto responder intact. I have included a zip file with the smtp details and the ftp details and how to test the form
