Build me a form on sharepoint

Hi Sir/Madam, I have been working on Sharepoint for 7 years. I have experience in Sharepoint installation, configuration, development and integration with 3rd party tools. I have completed many kind of projects that include the webpart, timerjob, event handler, workflow and masterpage/layout. I have developed lots of apps like audit management, document management, internal reporting. What sharepoint version do you use? Please let me know if you have any question.
