Create a php/html report from a sql query

AppGini knowledge a plus! I have a database which houses several tables, consisting of employees, their titles, and what brand they work for. I then have a table which houses various training programs and another that lists which training programs are required by which brand and for which position. I need a report that will display what training is required for each employee based upon which brand they work for and what their position is. This report should calculate whether a training is overdue based upon the employees hire date and the number of days a training is due after hire an place that in the table that's generated (see mock-up). This report should have check-boxes and a notes field (see mock-up) which allows the end user to "check-off" that a training has been completed and add any notes. This data should feed into another table in the database for housing/retrieval when this report is ran so that we can track the trainings that have been completed.
