Modify 2 Page PHP Website Showing Accumulated Time of Activities

Currently the program calculates and shows the accumulated time and cost of a repeated activity. It also lets you see how the accumulated-time and accumulated-cost are impacted when you reduce the activity by a percentage.

This job merely organizes and display data by feeding existing methods (already coded and tested) input fields then displaying results on the webpage.
On the first page:7 fields are input and submit button is pressed causing the 2nd page to be processed.
On the 2nd page calculations, based on data from page 1, are shown
Currently there are 6 fields. For this new job 2 text input fields, 1 radio button, 1 date field and 4 checkboxes need to be added.
ADD FIELDS:
Date of Birth (this could be date field)
3 Text Fields: User Name, Email, Activity Name
Radiobutton: Male Female
Checkboxes that say:
Show 10% reduction
Show 25% Reduction
Show 50% Reduction
Show 100% Reduction
On second page:
Top of page shows activity name, time it takes, etc(see attached sample report).
Next display a table for each show-reduction-percent checkbox selected. In each table display accumulated time savings for
1 year, 5 years, 10 years, 20 years and a special number
And SPECIAL calculation
SPECIAL calculation:age is calculated based on date-of-birth entered
Years-Special = if male it's age - 88
Years-Special = if female it's age minus 87
Then the Years-Special number is fed to method that determines the accumulated time and cost of an activity
Once the - the resulting number is added as last column on the table
