Please see attached excel sheet, which has three tabs, Front desk, dispencery, clinic. I need to make a data entry page for each category. each page should have its on user name and password and a admin account to open all three pages and edit/delete option. these are very simple excel calculation, which needs to be convert in to web format. on a weekly basics , person responsible for particluar department will enter their data and admin will login and run the report. In excel sheet you will see, data entry only done in "white back ground". so those are the only editable textbox, rest of them will be greyed out. on data entry page ,person should be able to see what they entred last week but wont be able to edit. only admin can edit . Admin should be able to run weekly, bi-weekly, or monthly report as per his/her choice. however, data entry can only do on week by week basics. so, starting week day must be sunday to saturday. so entry date is not sunday, data enrty/admin should get a warning.
