I have installed/configured a PHP/MySQL script from "http://www.gentlesource.com/comment-script/". It enables users to leave comments on a webpage, comments can be edited from an admin interface and includes a module for CAPTCHA SPAM prevention.
Current form fields are: name, e-mail, homepage, comment title and comment text
I need the scripts modified as follows:
(1) desired fields: name, email, employer name, employer state, comment text
(2) comments can continue to be modified/deleted by the admin (don't break existing functionality)
(3) visitors can view employers (and corresponding comments/database info) by "State". For example, suppose the database has 100 entries with two from each of the 50 states of America. If I want to only review comments made in Texas, I should have the ability to choose that and only see the two entries for employers located in Texas. The desired state can be sent (as a form post) from the requesting webpage if that makes the implementation easier/affordable.
(4) This is a bonus/optional requirement; please indicate if you are also able to implement this capability. However, it IS NOT required but is just a desired feature if possible. The requirement is to incorporate a ranking system that allows visitors to rank employers (1 to 5 scale is acceptable) and have those rankings visible to all visitors.
