Website redirect Script Creation Server based with user interface

I am looking for a website redirect script to be created.
The script will work with a database on my server and will allow me to login and access a user interface where I will be able to create new redirects - edit already created redirects and delete redirects.
When creating redirects then I should have the option to
name the redirect
add url to where it should redirect to
I should also be able to add conditions for browsers and countries
so for example I will be able to add url and say that if traffic is from mobile device then to send to URL X and if traffic comes from USA then send to URL X and if from Japan send to URL X etc
Options to select countries and regions is also import
so for example if traffic is from united kingdom and from the city Birmingham then send to URL X etc
The redirect code generated will be added to static html websites where we can add the code via <script> tags and it will also be used on sites such as free wordpress blogs, google sites and tumblr where we will need to add it in the form of an iframe
This script will be added to a windows server but you can select the language of your choice to create it in - javascript- php - .net etc
