Wordpress Formidable. Fix geo-location output so that each Formidable form supplies it back to the admin.

We are currently having an issue getting geo-location as well as other data from formidable forms via email when the forms are submitted. An example output of what we are trying to achieve is shown here the document along with other details of how we can continue to pay you on an on-going basis after this issue is resolved. At the very least, the 27 Main Form should produce geo-location and other such as what is on the form itself, as well as the Website URL, Referrer URL, and Browser/OS should also be listed. Currently, it is the geo-location data that seems to be the hardest to represent. The alternating colored rows seem to be achieved by default and it should continue to be displayed that way and replicated so that all the forms return this geo-location and other data to the administrator when the form is completed.
