Initial Contract - Subcontracting - Health and Beauty indistury

I would like to be a subcontractor for a plastic surgery and sell their services under my company that sell beauty products, I would like an initial contract format that include the information listed below

• Field : healthcare industry

• Object: bid proposal to subcontract with a plastic surgery

• Subcontract Location Company: several countries in EU

• Aim: convince the surgery to contract with our company that sell beauty products to add the Plastic surgery services to the services we sell.

• Packages will be sold first by the beauty company directly to the customer, and the beauty company take in charge wiring the money to the surgery – (minus) 30% on any deal

• The plastic surgery will ensure that the price provided for any intervention has to be at least 35% cheaper than any other competitor in the region of the customer.

• The beauty company (me) take in charge the initial blood tests requested by the plastic surgery and the follow up after-care

• The beauty product company is requesting exclusivity for this deal for a fixed term of 3 years
