Simple MVC project with SQL Backend.

See attached files. One is the form to be turned into a webform. The other page has details on the DB.

CRUD operations on the Form1. Everything on the paper form gets saved to a SQL server table "Form1" on submit.

The webform can be edited.

At this point nothing else. Not complicated.

Name the PDF fields the same as the SQL Field but with a prefix of "PDF_"

so... Name, PDF_Name. Etc.

Would prefer someone who can make this metro/win8 styled and good for tablets and phones as well as desktops.

Im trying to make a bigger project but I need to make sure someone understands what I need first.
