Javascript with MySQL -- record checks and inserts

Input variables are PIDNumber, LastName, FirstName, DateOfBirth, OrderId, ServiceDescription, ServiceReason, RequestorIdNumber, RequestorLastName, RequestorFirstName All in javascript: 1. Check/update a `person` table in MySQL database against some logic: a. If PIDNumber and LastName and FirstName and DateOfBirth match existing record, then exit and continue script. b. If PIDNumber does not exist in database, then create new record in table with PIDNumber, LastName, FirstName, and DateOfBirth. c. If PIDNumber and FirstName and DateOfBirth match existing record, then update LastName record. d. (etc) 2. Create record in `orders` table with foreign key to person table. Match "service description" string against table of service description strings and uniquely identifiable indexed values.
