The most useful thing I can tell you is that "open world" should be the last decision you make, not the first. Open-world content cost scales with area. Every square kilometre needs terrain, props, streaming, LODs, navmesh, population and testing, and none of that makes the game good - it makes it big. If the core loop is not yet proven, that spend buys nothing. Studios build the loop in one small area first and expand only once it is fun, and that ordering is why some projects ship. So milestone one is a vertical slice in a single hand-built zone: first-person movement and combat that feels right, inventory, one complete quest from trigger to resolution and reward, and a save system. That is playable and it answers the question that decides everything else - is this fun before we spend on scale. The architecture that matters at this stage is data-driven content: quests, items, dialogue and NPCs defined in data rather than hardcoded in scenes, so a designer adds content without touching C#. Get that right and later regions are content work. Get it wrong and every new quest is a programmer task.
