Risks, Actions, Issues and Decisions Databse

Currently have a Project Management Risk database. It needs to be expanded so that as well as recording risks it also records: Actions, Issues and Decisions So in total it will be a database that will allow Project Managers to record: Risks, Actions, Issues and Decisions. Please look at the attached database to see what the database currently looks like. In the Risks part of the database I need to capture the following information: Risk Ref Number: For example “R1” Date Received: For example “30/09/06” Risk Type: This should be a drop down box allowing me to select the risk type. There should be an admin area in the database that’s lets me define the risk types. Very similar to the “Edit Priorities” feature of the current Issue database. So perhaps you could add an extra button here stating “Edit Risk type”. Risk Owner: This should be a drop down box, allowing me to select a name. This can be populated using the same data source currently used in the existing database. Risk Description: This needs to be a text input box, similar to the “comments” section in the current database. Impact: A drop down menu with High, Medium or Low to select from. Probability: This is a drop down box with High, Medium or Low to select from. Severity of impact: This is a drop down box with High, Medium or Low to select from. Proximity: This is a drop down box with High, Medium or Low to select from. Contingency: text box area to type comments into. Status: drop down box with Dead, reducing, increasing, no change to select from. Target date: calendar select option, allows project manager to estimate risk closure date. Progress: Text input box allowing Project manager to make notes with progress. In the Actions part of the database I need to capture the following information: Action Ref Number: For example “A1” Date received: calendar select option Action Description: : text box area to type comments into Action Owner: Drop down box selecting name Required by: calendar select option Action Status: text box area to type comments into Date & Progress update: text box area to type comments into In the Issues part of the database I need to capture the following information: Leave as it is but add “Rag” Status, which mean Red, Amber, Green. It allows a Project manager to see the status of a project. In the Decision part of the database I need to capture the following information: Ref number: For example A1 Date Made: Calendar selection Description: Text box Decision maker: Drop down box (populated from the same data source in the current “Contact” field of the existing database). Follow up actions: Text box Assigned to: Drop down box (populated from the same data source in the current “Contact” field of the existing database).
