Feedback Scroller

Need someone to be able to make something along the lines of what eBay offers regarding feedback, but customized to meet my specifications. eBay has a feedback listing called "my World" where it lists your feedback inside a scroll. It scrolls between feedbacks & allows you to pause on each one. I will not need the buttons to go to the next one or backwards like eBay has. An example is what's listed under "Latest Feedback" @ [login to view URL] with regards to what I'm talking about. I need something like this, but customizable to feature U-Ship feedback ratings. We need a section for Rating, Customer, Date & Item that will all have text fields. We also need to be able to have a section for Communication, Care of Goods, Punctuality & Services as Described where it can be listed as 1 to 5 stars. Then we need a feedback section which will be a bigger text field. For the right person this should take no more than an hour or two. I'm offering $100 for this project. I want the ability to be able to add in the text for each field & to have the option of selecting how many stars were left. So essentially you're creating the item so that I can constantly update feedback listings for a client. I want to receive an initial submission of your completed project. Then, if it's exactly what I'm looking for payment will be sent. Chances are it will need to be reworked; so I want to see a first submission before proceeding. I also need it to be easily placed within a website so prefer HTML/CSS because that's what I'm used to working with. DO NOT BID IF YOU CANNOT MEET OUR PARAMETERS. IF YOU COMPLETED THE PROJECT SATISFACTORILY YOU WILL BE PAID IMMEDIATELY. IF CHANGES ARE REQUIRED WE WILL DISCUSS NEW TIME FRAMES AT THAT JUNCTURE.
