Someone to create an online test

am looking to create an online test for measuring performance. I am looking to be able to perform the following as there are some specific requirements I need to be able to do: It would ideally be an online test Front End The candidate needs to be able to enter their specific goal at the beginning. The whole test then is designed to measure against this goal. i.e. they answer questions with this specific goal in mind. The goal needs to appear above each question as a reminder about the context in which they are answering their question There are approx 60 questions split into 10x questions across 6x categories Each question requires, as the answer, the candidate to score their answer as a mark between 1 and 10 e.g. 1/10, 4/10, etc Back End The test is broken into 6x areas, each requiring its own category score based on the 10 questions within it. This score is calculated by adding the 10x individual questions scores for each category together and dividing by 6x to give the overall average as a percentage. As an example if they score 5/10 across all 10 questions then the score is 500 divided by 10 = 50% average across the section. Some of the questions are written as a ‘reverse score’ so for instance if they mark 4/10 then the calculated score needs to be 6/10 in the scoring at the back end when the calculation is made. The six individual overall category scores need to then be added together and divided by 6 to give an overall % score e.g. section 1 = 30% 2=40% 3=60% 4=65% 5=80% 6=50% 30+40+60+65+80+50 = 325 325 divided by 6 = 54.16% overall score Depending upon the score ranges I would also want to be able to generate an individual report for each of the 6 categories based on a high/medium/low score range taking the overall category score range e.g. category 1 = 30% falls in a ‘low’ range. Ideally I would like to be able to take the previous point to an even deeper level and be able to make an individual report crafted around the individual question scores setting the high/medium/low ranges. This would make the test results more powerful and effective for the user. I would need the test hosted and available for either online individual purchase of tests or for selling a licence code (as well as multiple user licences) for multiple uses across a certain period e.g. 12 months. (though ideally to do both)
