This is page 1 of the site of 12 pages. It is a long 11 step form with confirmation page. It is then sent to an access database for storage and future maintenance would allow for calling previous submissions to auto-populate for increased efficiency.
About Me
I am a certified (CTFL) software tester full time. I enjoy projects on the side doing web development, including the design, documenting specifications, generating the css and jquery needed for these projects and figuring out ways to increase efficiency for end users.
