The app is based on the Google appsheet platform. It has all the capabilities as requested and further functionality can be added if more detailed analysis is required in the future then is currently required. The interface is kept clean so the users entering data have to go through minimal learning curve. It can be used on android / iPhone or even as a webapp on a laptop browser. I hope the screenshots give an adequate idea. A demo can be arranged as well if an email address is provided.
