I need a graphic created for a contest I am running. I've attached a 'sample' graphic to show what I would like the graphic to be similar to. Below are a list of changed I would like:

1. At the very top of the graphic, it should say 'Show us your RAD for a chance to win awesome Just Care More Swag!'
2. In the white bar that goes across the images, it should have the Just Care More logo (attached), as well as the words 'photo contest' with an image of a camera
3. In the blue bar below the images, it should say 'Purchase additional swag at store.justcaremore.com
4. Next to 'Snap' it should say 'Show us your RAD by snapping a pic with your Just Care More Jersey. Whether you wear it skydiving, rock climbing, hiking, etc., we want to see what RAD things you are doing! Don't have a jersey, no problem! Just snap a pic and show us what 'RAD' looks like to you!
5. Next to 'Enter' is should say 'You can enter by uploading a photo to the facebook contest form, or uploading a photo with the tag #JUSTCAREMORE on Facebook, twitter, or instagram. Share your photo to get as many likes as you can!'
6. next to 'win' it should say 'Photos with the most likes will win some awesome Just Care More swag such as a Just Care More hat autographed by Jamie Leibert, Manicorn Socks, Stickers, wristbands & more!'
7. For the photos, i've attached screenshots of photos that can be used. They need to be cropped. There will be one image with an ipad in front of it, similar to the sample graphic, almost as if the ipad is taking a photo

Format: the photo should be created in photoshop, as I will need the jpeg and .psd files.
Size: the photo should be the same size as the sample graphic
