create a survey by using sharepoint 2010

i'm looking for a very good coder to create a survey with only one question and 4 possible answers.

the result should show me how many user have voted for the first answer, for the second one ..and so on ...

the result should also display a time intervall (one week, 2 weeks and 4 weeks )
