Find US public company data on SEC EDGAR and fill in spreadsheet -- 3

I need help with looking up and populating select data on a sample of US-based publicly traded companies in the attached excel spreadsheet. I am looking specifically for the following fields:
CFO name (CFO stands for Chief Financial Officer)
CFO age
CFO date of appointment
Company headquarters location
This information can be found in SEC Edgar filings 10K and/or DEF14A (Annual Proxy), which can be looked up by using the company ticker symbol in the first column or its name in the second. The Edgar website address is [login to view URL]
Strong familiarity with SEC Edgar database is a must.
