Flash quiz desktop application (flash projector)

I need a small flash quiz developing. The output should be flash projector (.exe), PC-format only. The application should open fullscreen with the fscommand, and should be optimised for 1280 x 1024 monitor resolution.
The development environment needs to be flash CS3. The quiz should initially capture some user data (name/email etc) and save this information to a csv file on the user’s computer. If another user completes the quiz, the csv file should be added to, and NOT overwritten.
The user will then be presented with some information based on a powerpoint slide format, followed by a multiple-choice question. There will be a total of 10 questions, and the quiz should at the end inform the user their score out of 10, in terms of correct answers.
There should be transitions/animations between screens. Please see this powerpoint file for story-board of how the quiz should flow, and please see the ‘notes’ section in the slides for additional information: [url removed, login to view]
I do not have all the finalised information at this stage regarding the slide content and the question content, so I need to be able to edit the .fla file and add this in when I do have it.
I will provide graphical style for the quiz. I will require the .fla file sending back to me, and the .fla should be compatible with flash CS3.
