Looking to develop google web app for sales process automation based on google sheet and google scripts along with google web app.

Need to create simple google app with below workflow (to create and submit Minutes of Meetings done by sales team):

1. Log-in page (need to restrict log in based on google account authentication) - only allowed uses should be able to log-in

2. The next screen should have a form to submit minutes of meetings with below fields:

-Date of meeting

-Time of meeting

-Place of meeting

-Type of meeting (Pitch/Requirement/Reconnect)

-Client organization name (the user should be able to choose from list of clients from a google sheet) - required field

-Client contact name (the name of the person from the data of the sheet - user should be able to select it as he types name) - required field

- Team from our side (that is name of persons from our side, user should be able to add team members from a google sheet) - required field

-Add any other names with description of the their role (like vendor, client's vendor, etc.) - optional field

- Points discussed (this should be open paragraph filled, each point should be separately added) against each point there should be field to add due date for that point, and another field to add responsible person name for the point (names should be from either from the team who was involved in the meeting from client side/vendor side, etc. or from our side)

-Submit button (once it is submitted, an email should be sent from that user to himself with an attachment of all the details in PDF format) (file name should be MOM_OUR CONTACT NAME_CLIENT NAME_CLINT COMPANY NAME_MEETING TYPE_DATE OF MEETING)

The app should be secure (not public), any information submitted here should not be accessible by any other person. Basic hygiene of development should be maintained (for security, privacy, etc.)
