Customize my tumblr

Want to use my tumblr as main website for my band. I've bought a custom theme but still can't get it exactly as I want it... I think it should be fairly simple for someone who know what they are doing. Here's what I want:

-Header Image with social media buttons in corner

-Navigation bar that only has link to "Store" and links to my bandcamp page which opens up in a new window.

-Sidebar on left side with these elements divided by a line:

- picture of album cover (also links to store in new window)

- instagram feed that shows 4-6 latest posts from ig

- "join mailing list" button that links to a sign up form that pops up on top of the page,then to a thank page that I would also like to pop up if possible. I have an autoresponder in place and would like to integrate it

- "Contact" button that links to a contact form that pops up on top of the page, then to a confirmation page that pops up. Will send e-mail to an account I already have set up.

**Also want the tumblr page to be located at www."bandname".com instead of www.bandname.tumblr.com. I already own the domain and hosting but need help redirecting it.

** Would like the bandcamp page to be located at www."bandname".com/store or store."bandname".com. Bandcamp is already setup.
