research tool

Web based application that administers questionnaires to research subjects. The program does not need graphic design but needs to look ok for people.

The tool needs to combine 4 questionnaires:

**1. Demographics**. Starts with general information, subject should have the option of printing the first page with explanation. 31 question (some check, some type, some scroll). See word doc intro and background questions. The order of the questions is always the same.

**2.** 17 question for each of 7 emotions. Total 119 screens. The first item asks for the person to narrate an event in which he r she has experienced the emotion. This narrative should be recorded and appear in the following 16 questions about the event. The 17 questions are exactly the same, but in 1,3 and 4 you have to change the underlined text with the actual emotion. The order of emotions should be randomized by the computer for each subject and the program must record in which order each emotion was asked from each subject.

**3.** 60 to 70 questions the person needs to check on a likert scale. Ex. 0 Strongly agree 0 Agree 0 Do not know 0 Disagree 0 Strongly Disagree

Each question should be displayed in a different screen and it should be set up so the subject does not make mistakes when answering. For example is yes or no question the person can only select yes or no, etc.

All answers MUST be recorded in a database that I can access all together or per subject. Each subject should be identified by number.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
