Custom Form + Price Calculator (No E-Commerce)

Details are as following.
Form needs to be in two parts.
Part 1 - Price Calculator.
There are around 10 types of products, each with about 50 variables to choose from. I think the following example illustrates it as best as I can.
Business Cards
Weight: 280gsm, 300gsm, 350gsm, 420gsm
Coating: Unocated, Matt Celloglaze, Gloss Celloglaze
Quantity: 500, 1000, 1500, 2000, 2500, 3000, 4000, 5000, 7500, 10000, 15000, 20000
So for instance, a client can choose "300gsm + matt celloglaze + 5000" and it will give a particular price up on screen, or they can choose "420gsm + matt celloglaze + 1000" and it will give a different price.
If a refresh button is needed to be pushed thats not a problem.
Part 2 - Order Information
If a client decided to continue with the order, the information must be remembered on the next page of the form. (It can be an "accordion" if you like, so for instance the price calculator slides over to reveal the contact information for the person creating the order)
After the client hits submit, the form will be emailed to a specified address.
No designing will be necessary, just pure coding. Will need to fit in with current CSS styles of the website.
Willing to hire from anywhere in the world, providing you can speak fluent English.
I have also selected "PHP & Javascript" as I believe these will be the best languages to work with, however, if you would like to suggest another I am open to that.
