917 PHP/MySQL REports

We need to create reports for this project. I am going to need to be able to print this out in some kind of professional format. I simple excel export is not going to work. I think that we can use the same layout for a few of the reports. PDF document would be the preferred file type for now. If you can think of something easier or faster, but looks professional like that, please let me know. We need reports for the following fields A. State – Need the report to list out all the states that are in the database records. Tell me how many records have each one of the states and then a total column of all records on the bottom. B. Zip Code – Same as above, but with the zip code C. Field of Interest – Same as above, but with field of interest D. Source (Where did you hear questions?) – Same as above, but with source E. List of New entries – Client will need to be able to print off a report that shows all of the new entrants sense the last report was pulled. This report will need to be sent to there members by fax, email, mail and any other feasible way. This report is going to need to list the contact information of each new record and what areas of interest they have. Will need to be able to export this report in both excel and PDF.
