Very Simple Database in MSAccess and MySQL

I am looking for an expert on MySQL and MsAccess.

The project is very simple.

Structure: 3 tables with few fields, only linked to each other by one ID.

Forms: 1 simple form with all fields and a "search button"

Report: 1 simple report with the returned data.

This is a test, and the winner will be a hot candidate to design the entire database which we be a bigger project.

Please only reply if you are an expert, even though the job is simple. I attach an excel sheet with more explanation.

Dear Providers, as you noticed I am not an expert in this field. So before we move on to the MySQL/PHP webbased DBase, I need a test in MSACCESS.
I also need a form to enter new lines, or import them (manually) from excel.

As the project became even more simple, I estimate an expert to be able to do it within 1-2 hours max. Therefore no need to bid more than $30.
