Wordpress Squeeze page design/tutorial

This is what I want:

-Create a wordpress page (not website) with custom design/theme

-Teach me how to modify elements (we will communicate through skype)

This is a very simple job. I want to create something like this:

Nothing fancy, it has a nice background image and a nice opt-in form. I want you to create something like this for me and teach me how to do it.

I already have a website with a nice theme, I don't want to change that. I just want to add this page and is completely different from the main theme. It will work as a squeeze page.
