2 Website Contact Forms

Please read entire posting, or don't bid on this project.

I need 2 forms designed. They should have a similar look/feel/functionality to the form below, but of course matched to my site:

Code must be self-contained to be pasted into a Joomla article - nothing to be added to the {head} section. Form must be standalone, NOT integrated into Joomla in any way. Forms will email information only - no database needed.

Both forms will have several options:

- a yes/no question which will determine what the next fields will be (the rest of the form will only appear when an answer is chosen, different options depending on the answer)

- the form will be sent to a recipient determined by pulldown choices

- form results must be sent well-formatted, but using standard text

- CAPTCHA must be relatively simple, with a refresh option, not CaSe-sensitive

Form must not use Flash or Tables, and must work in IE6-7-8, FF2-3, and Chrome.

This project is a rush - please provide DIRECT links to forms you have designed in the past in PMB (or explain why you don't have any), or your bid will be ignored. Payment by escrow, to be released once form code has been tested live on my site. Budget is under $75.
