Interative Questionnaire - Flash or Java

I have a facebook page that i need an interactive service selection tool. My project is for creation of an interactive questionnaire which contains 18 questions to determine the service which best suits our clients. Requirements: - Creation of a flash/java solution (would prefer a solution which can be utilised on iPad and iPhone) - Will contain existing branding - Highly polished graphics design skills. - Integration into Facebook Page Example of system: Question 1 Are you going away? (Drop Down Results - Yes or No) If answer equals Yes, next question (Question 3). If answer equals No, next question (Question 2) Question 2 Is your pet(s) alone during the day? (Drop Down Results - Yes, I work full time OR No, have someone at home most of the time OR Always home If answers equals Yes, next question (Question 5) If answers equals No, next question (Question 6) If answers equals Always home, next question (Question 7) And so on.
