I need someone to write a script to push a value (page URL) that someone clicks on a CTA on to a form. The form will live at it's own URL. The purpose is there are 50 some odd dynamic pages with a similar CTA. Based on which page they click on the CTA on, I'd want the form to pre-answer a question, which will customize the form. I would also be open to, and prefer even having the form live on each dynamic page, but somehow having the URL inform the answer to question #1 upon loading.
