Hi, We have a client who has an existing website in PHP with a fully functioning client area already working, and requires the following: 1. Once a client is logged into the site, a form will appear with the following fields; Date (Calendar Field) Clinic/Patient Number: Text Field Patient Name: Text Field Therapist: Text Field Disorder: Text Area #Shocks: Text Field SWT$: Text Field All fields are required fields. Once the form is saved the form data needs to be stored in an excel spread sheet with the same field names. Finally we need an export link: [login to view URL] Thank you
