Questionaire added to site with paypal addition

We need a questionaire that we already have in php uploaded to a site (it is in a 4 page format), it will have its own tab , we want a couple of changes to questionaire then we want a banner on each page of site in best fit locations (6 pages at present) and have the ability to change wording on banners Banners must fit in with site graphics and colours and looks nice but stands out with text that is easily changable , Once a banner is clicked on ,This will link to Questionaire page (questionaire should have its own tab) will open on page with a introduction to questionaire and a button to take payment which will link to paypal ,( the questionaire should not be visable at this point) once paypal payment is taken client will be returned to site and to 1st page of questionaire the Once questionaire is finished and submit button on last page is pressed it emails us and enters details into database (which is in database file in questionaire) . Also at same time sending a thankyou email to the submitter of the questionaire That is what we want please only bid if you have experience of graphics , php, paypal etc we will advise more detail after bidding process
