Javascript Added to Existing Membership Form

I'm looking for someone to add Javascript form field validation and a calculation to an existing membership form, which was created using FrontPage. The form can be seen at [login to view URL] Fields to be validated as user required are... 1. Name 2. Address 3. City, State, and Zip Code 4. Daytime Phone 5. Email Address 6. The user is required to check at least one checkbox in the "Yearly Membership Categories" section. (This doesn't include the "Anonymous" checkbox, which is not required.) 7. Also in the "Yearly Membership Categories" section, if the "Lifetime Member", "Lifetime Benefactor", or "Lifetime Leader of the Pack" box is checked, the user is also required to enter a dollar amount in the corresponding text box (and I think this text box has to be validated to accept only numbers because it's included in the calculation). 8. If the user checks the gift membership box, then the gift membership Name, Address, and City/State/Zip Code boxes are required. 9. If the user checks the gift membership box, then the same rules as mentioned in #6 above apply to the "Gift Membership Categories" section. 10. "Method of Payment" should be a user required field. Then the calculation... 1. A calculation is required for "Membership Total", which would be the total dollar amount of all "Yearly Membership" and/or "Gift Membership" checkboxes/text boxes the user has selected. Also, I would like this dollar amount to be formatted to two decimals, please. That should be it, except I've seen address, phone, and Email form validation that somehow checks to see if what a user enters in these fields is probably an accurate Email address, etc., and I thought it would be nice if that type of validation was on this form.
