Using PHP and MYSQL we require a login system that allows one time use numbers to get surfers to submit information. Once the information is submitted that number used to log in cannot be used anymore, make it in active. Two pages are required the first is a login page with number to be submitted, the second page will be a form where once submitted the information is stored in the mysql database and an e-mail is send to an address, at you point you also want to change the active flag on the login number. The login page does not have a password just looking for the number that is stored in the database.
