PHP Catalogue Request Form help

I have recently redesigned a website for a client, updating a previous site. The client has asked that I retain a catalogue request form that is on the existing site. I transferred the form across to my newly developed site and it has been working fine until recently. The form sends some of the field data by email to the client but none of the check box field results appear in the email. I require some assistance from an experienced webform developer to examine the source code and write some code that effectively sends ALL the fields by email to my client. The form has already been created and there is already a script in place. I believe it may just need checking over and tweaking. Many thanks
