Need Help Setting HTML site Up

Hey everyone 1st let me tell you i am trying to get my site up webintermax dot com up , so what i need help with is putting in form codes and getting the site up, you can go to my other site kalobjames dotcom and click the live help once you take the job so we can get the site up to my standards, you basicly have to just get my logo put on the 5 pages and some code ill send you in text file to put within the site. thats all i am only willing to pay $15 for this job as it can be done less then 15-20mins MAX, but i have other jobs such as script install if you want those jobs ill pay accordling thanks
