Socialengine 4.50 Contact form with automatic filled fields from profile and log page

Hi, I require a freelancer who is experienced in Socialengine (4.50) development. Please only experienced SE developers bid as SE is quite a comprehensive social website framework and I would like to stay in touch with the chosen developer for future (larger) projects. Here are the requirements (this should be done as complete plugin or files that I can upload and install/override etc. as I need to move my SE website soon so I can always install this on a new SE in the future): I require a contact-form widget(button) that I can add via Layout Editor to a profile. (So basically a new HTML Block and lead to a new page). The button should say "Apply" and I need documentation where I can change the buttons colour in the future. I use two profiles in my SE site (plugin AdvancedProfileType) - Students and Universities with both different profile fields. As the contact form widget will be available via layout manager, I can add them to one of the profiles namely Universities. So when a student accesses a university/school profile he or she will see the Apply button (see image 1 - button example ) After clicking the Apply button a new page will open which contains an form (html, jquery etc. its up to the developer how the visual design of the form will be - maybe just as a simple grid layout as in image 2 or a nice jquery form) as illustrated in image 2. Some fields (3 standard fields) in this form should be automatically filled from the students profile page (which ones are indicated in red in image 3 ! ). All the other fields need to be filled by the student manually. There should be two buttons on the form website at the bottom (see image 2) "save" and "save & send". After the student has entered all or some manual fields he can press "save" and all data will be saved in that form. After pressing "save & send" the form will be send as an HTML email (as it looks on the form website) to the University/School ( the email address of the university is taken from the university profile/account). The form should be send as HTML as it is displayed in SE with email subject title "application". Also when a student presses "save & send" in the students activity feed should a message appear [name applied at name of Uni] !Important! Once a student has pressed "save" the form will be saved and the content will be available to use for other university/school applications. For example, student A clicks on apply on uni A profile, then fills the manual fields in the form and clicks "save & send" then the application form is send as an email to uni A but also saved. Now student A clicks apply on school B profile then all the previous entered information is still there and can be altered if desired or simply click again on "save & send". This is so that students do not have to enter their information every time again when they apply for a new uni. Also then students click "save & send" then there should be a tab/widget in the students profile which says "My applications". Under this tab/widget there should be a table showing the name of the university/school that the student has applied for (please see image 4). This tab/table should 1) only be visible to the student him/herself and administrators. The full table example is shown in image 4. Fields are: Applied at; Date; waiting for response; offer received; enrolled; When a student clicks "save & send" on a application form page then the table should automatically list the name of the School in the field "Applied at" and also the timestamp as date (just the dd/mm/yy is fine). All other fields are ticks which can only be activated by an administrator accessing the My applications tab of a student profile. Either the admin activates a tick then the tick is displayed or nothing is displayed. Documentation is important please! That is it ! :-)
