264518 Data entry & basic html report

I need some basic data entry work done and a php file that pulls the data and formats it to fit with an existing website design. More detailed information can be seen in the attached file. I am looking for someone who is available to start immediately, so be precise in your bids as to how long these changes will take you. I will expect the project to be completed by the day you specify. Bids placed that are not within budget will be removed. I will place payment into escrow upon start of project and release it upon submission of the required links. I expect the project to be completed accurately and on time.
