Simple check-box & search page

I'd like a simple web page and database I can edit myself. I want a top frame to add my own banner, two side frames so I can put a table in each for advertising. In the middle I want text box so people can type in a web address, a sample set of tick / check-boxes (5 will do) so people can tick them, then a submit button so the web address and ticked boxes will be submitted to a simple mysql database of your creation. The time and date of the submission should also be included on the database, next to each entry. Next to the submit button, there should be 2 options to search the database using the check-boxes. One search button should be to search for 'all' (which means that it would only bring up results for urls that match ALL ticked check boxes). The other search button should be to bring up results for 'any' (which means bring up urls that match ANY of the ticked check-boxes). The search results should appear in a new page that has a frame down each side set up ready for advertisements to be added. Where there is more than 1 page of results the number of pages should be displayed (like Google does), and users should be able to choose which page they want to jump to. Please bear in mind when bidding, the database code and php web page code need to be VERY simple, so simple even I can rename, or copy and paste check boxes etc from that point on. The one who submits the easiest to understand code, but gives best search results, will be chosen as the project winner.
