HTML5 Solution that will play and record mp3s

We are currently seeking someone who has the ability to create an HTML5 application that will allow us to create interactive questionnaires. For example, we upload an mp3 asking the question "How was your day?" the user will then be prompted to record their response "It was fine" when the user is done the recordings will then be saved and then onto the next question until the questionnaire is complete. Also will need a management portal so we can keep track and listen to users recorded answers. We're open to whatever solution is quickest and most cost-effective. Let me know if you have any questions.
