Portal to enter number and then write to file with formating

PHP 5.6 Web portal to enter phone numbers in and output to to a file as per the format (.call) below. 1. 1 or more numbers can be entered into portal at a time 2. 10 ditgits only per number XXXXXXXXXX 3. Number must start with 02, 03, 07, 08 or 09 only 4. If more than one number entered into portal wait 1 minute between output of .call file 5.
