A Simple CRM Software -- 2

I need a simple CRM software, which will run on a server and users can login the server via web interface. Software has to store data, get data and search data. I have 3 main entities to store their data: Customer, Potential Customer, Coder Customer has following fields: Company Name Company Sector Address Phone Tax Office Billing Address Contact Name Referrer Name Order Due Date Assigned Coders Description(I need an auto date appended time when a description written down) Notes Coder has following fields: Name Address Phone Price Bank Account Details Potential Customer has following fields: Company Name Company Sector Address Phone So I need a database designed to capture these entities in MySql, and a web gui to insert update delete and search data. You can make suggestions about changing programming languages or database, but I think PHP and MySql suits well. Additionally there will be two types of users, admin and normal. Admin user has privilege to add user delete user and monitor insert update delete operations. And localization option would be a big plus, I don't want to go over all the source code to translate in another language. For example, it should be possible to translate software by adding a language file. I want a minimalistic and easy to use design, fancy guis not needed. And I think 3 forms for inserting, 3 forms for searching necessary, when an entry selected there should be an update button, leads to update form, and there should be a delete button to delete the entry. You can also propose something else for gui, with same functionality, if you think its better. Notes: I want the source codes as well. I may modify and redistribute it later on, so you should not claim any rights on the source code.
