Script Code for questionaire

We require a programmer to script some code to drive a 15 part cosmopolitan style questionaire. Based on mostly multiple choice the code would compile a score and report back to the user, it would automatically email the results to a set recipient. we had though of this as a combination script / cold fusion code, but are open to suggestions. we would do all design work. get back to me with a rough estimate on time and cost and cv daniel
