Azure SQL Database + Data Entry via Simple Website - repost

We need a SQL Database developed and hosted with Azure. Also, related we need a simple Data Entry Website for human input of this data we want this website written in ASP .Net and hosted in Azure also. C # is the preferred language for this ASP .Net development We have an Azure account and will add whatever is needed there for the project. This is somewhat a very simple, small scale, text book style data base. I am attaching a bit more detail on the database. All data entry will be via Web Entry form. In a next phase of this project, this same SQL database will be accessed by apps written in C# displaying this data in a read only manner Prefer developers with significant experience in .Azure hosted SQL development, ASP .Net using C#, and Database integrated Apps development
