269400 Webdesign Quote Form + css mod

First, the CMS system that is used for the site I want the below done for is CMS Made Simple. You should be familiar with this CMS system if you are bidding. Here's what I need done. #1 - Need a form created that is similar to the one here: [login to view URL] The "form builder" module within CMS Made Simple should be used for this. The form will be located on this page: (It also needs to match the design of the rest of the site). [login to view URL] #2 - Need a "block" added. Probably just some CSS koding. Example image below.
